The question we get most isn't about A1C. It's simpler, and harder: what do I actually eat? The internet hands you six answers before breakfast. Fruit is fine. Fruit is sugar. Go keto. Stay under 50 grams of carbs. No, 100. Don't restrict at all, build carb tolerance instead. Every one of those comes from someone confident, and they can't all be right. But you don't need to referee that argument. You need a way to judge the food in front of you.
Here's the fact that shows why the arguing never settles anything: corn flakes can have a higher glycemic index than table sugar. Cereal scores around 83 on the glycemic index, sugar around 64. That's no reason to spoon sugar on your breakfast. It's a sign that "is this food good or bad?" is the wrong question, and that most of us reach for the wrong number trying to answer it. Two numbers, two different questions. Glycemic index (GI) tells you how strongly a fixed amount of carbohydrate tends to raise your blood glucose. Glycemic load (GL) adds how much carbohydrate is actually in your serving. GI describes the carbohydrate quality of a food. GL puts that number in the context of how much you actually eat.
Why carbs get blamed for prediabetes
The blame makes sense. Carbohydrate has the most direct effect on blood glucose, and in prediabetes your body doesn't respond to insulin as effectively, so managing that glucose becomes harder. Blaming carbs isn't crazy. It's just imprecise.
Here's what a fast carb can do once you've swallowed it. With little fiber to slow digestion, glucose can enter the bloodstream quickly, prompting a larger insulin response. For some people, that rapid rise and fall can leave them hungry again a few hours later.
Compare two breakfasts:
Plain bagel: GI ~72, about 55g of carbs, ~290 calories, 2g of fiber.
Rolled oats, 1 cup cooked: GI ~55, about 27g of carbs, ~165 calories, 4g of fiber.
The bagel isn't different on one count. It hits several at once: higher GI, roughly twice the carbohydrate, more calories, and less fiber.

Packaged food makes this harder, as you know — sugar hiding under a dozen names, "whole grain" stamped on bread that barely qualifies, "low carb" on a wrapper whose contents behave nothing like the claim. Switching to whole foods cuts through most of that in one move: no label to decode, no marketing to see past, and you get the fiber and protein that slow the whole process down. That alone puts you ahead of most people managing prediabetes. It just leaves one question open — sweet potatoes and lentils are both whole foods, and they don't land the same way. Which one, and how much, is what these two numbers answer.

How the glycemic index works (and where it falls short)
To compare foods fairly, researchers typically give people a portion containing 50 grams of available carbohydrate and measure the blood glucose response against a reference food, usually glucose, set at 100. Under 55 is low, 56 to 69 medium, 70 and up high.
That fixed dose is what makes GI fair — and it's why GI works best as a comparison tool. Use it to rank foods you'd eat in similar amounts: rice against quinoa, instant oats against steel-cut. Where it breaks down is on portion size, because a food can score high even when a real serving carries very little available carbohydrate. So compare with GI, but decide with GL.
How to calculate glycemic load
Glycemic load closes that gap with one line of arithmetic:
GL = (GI × grams of usable carbs per serving) ÷ 100
Under 10 is low, 11 to 19 medium, 20 and above high.
Watch what it does to three foods:

Lentils, boiled | ~25 | ~24g | ~6 | Low |
Pumpkin and white rice have almost the same GI and nothing else in common. Then compare the rice and the lentils — same size bowl, eight times the glucose load. That swap is the whole lesson.
What glycemic load still can't tell you
These values are averages from fasted people eating one food alone, which is nothing like dinner. Ripeness shifts them, so does processing, so does cooking time. Cooling matters too: chilled potatoes lowered post-meal glucose and insulin in women with elevated fasting glucose, though the same trick tested on rice hasn't held up consistently.
Then there's you. Among 795 people eating identical bread, the highest-responding tenth ran more than five times higher than the lowest. Treat these numbers as a starting hypothesis, not a verdict.
How to use GI and GL in everyday decisions
Picking between similar foods — rice or quinoa, instant or steel-cut oats — is a GI question. The portion is much the same either way, so only speed changes.
Deciding how much of one food — "can I have potatoes?" — is a GL question. The answer can be yes, at half the portion you had in mind.
Building a plate at home: lower-GL choices are a useful starting point, but portion, meal composition, and your individual response matter too.
Order matters as much as content. In adults with prediabetes, eating protein and vegetables before the carb cut the glucose peak by more than 40% — same meal, same calories, different sequence. So: vegetables first, protein next, carb last. Read: Carb cravings and spikes? Your protein solution
